I know where to get it in pokemon platinum/diamond/pearl.
but not in soulsilver

In Blackthorn the house above and to the right of the pokemon center there is a person in there that will tell you if your pokemon need more help or not and there you will get the effort ribbon hope this worked!

It's the house to the right, and when you enter, the lady to the right will evaluate the Effort Ribbon for the first Pokemon in your party.
