There’s an automatic system in the site that adds a question mark to the end of every post title, so it’s detecting this and adding it back each time.
Obviously the question is already there, but there’s no real way to stop this happening — it’s a bit flawed. My best recommendation is to put the bracketed phrase behind the first question mark.
