How do i get the pokemondb app on my phone?

It's not really an app. You don't install it like you would to play Angry Birds or something, lol.
All you have to do is just is go to this link: m.pokemondb.net.

vanilts is right, you just visit m.pokemondb.net in your phone's web browser.

However, on some phones (such as iPhone) you can "install" the app and add an icon amongst your regular apps. On iPhone you click the middle button at the bottom then click "Add to Home Screen".

You should see an icon like this:
