In Pokemon Black/White the Pokemon with alternat efors are;
Basculin (lol), Darmanitan, Deerling/Sawsbuck, Meloetta, Giratina, Shaymin and Genesect
Basculin just go capture the red/blue one :O
Darmanitan changes depending on the amount of HP left. You'll have to get one of the ones who sit outside the Relic Castle or from the Dream World and when they're at 50% health, boom it's Sp. Def/Sp. Atk are switched around with Atk/Def
Deerling and Sawsbuck change according to the weather
Interact with the Meteorite in Nacrene Museum and Deoxy's form will change in the order of Normal Forme -> Attack Forme -> Defence Forme -> Speed Forme
With Rotom, go to the Department Store on route 9 and the lower right room should have some applicances (in boxes) which allows you to change Rotom's form
Giratina is done by recieving te Griseous orb from the Shadow Triad on the Cycling Road on Route 16 and giving it to Giratina
If you have Shaymin from the event and defeated the elite 4 fo to Lacunosa town where you can recieve the Gracidea to change Shaymin's form at certain times of the day
Genesect is done by having different drives and Meloetta is done by going to the Cafe in Castelia and learning Relic Song

I'll list the ones obtainable in BW:

Rotom - Heat, Wash, Fan, Mow, Frost forms. Go to Route 9, into the Shopping Mall 9, and go into a little room on the bottom right. There are some appliances there, and let Rotom play with them. The one you choose determines Rotom's form.

Darmanitan - Zen Mode (hidden ability; you need to give one of the statues a Rage Candy Bar outside Desert Resort). When its health is half/ below half, it'll change into Zen Mode. When its health is restored, it'll revert to its normal form.

Deerling/ Sawsbuck - changes forms due to seasons

These you can still get in BW, but they only change forms in B2W2.

Thundurus, Tornadus & Landorus -
>The kami trio change forms by using the Reveal Glass, which is only obtainable by capturing the Landorus in the Pokémon Dream Radar and taking it to the Abundant Shrine in Pokémon Black and White 2.

Kyurem - White & Black, use DNA splicers to fuse with Reshiram/ Zekrom, but only in B2W2.

Keldeo - Resolute form. Teach it Secret Sword at the pledge grove, but only in B2W2.

Form Differences - Bulba
