It appears that the one at the top is indeed missing a segment. The one at the Evolution Chart has all three though. It looks like the Three-Segment-Form tab just shows you the SV sprite of Two-Segment. When looking at the Sprite Gallery, there is no Three-Segment-Form there either. Strange.
