Just two forms? Well that would be:

Giratina
Shaymin
Cherrim
Basculin
Shellos
Gastrodon
Darmanitan
Meloetta

However, there are Pokemon with more than two forms. They are:

Rotom
Deoxys
Unown
Deerling
Sawsbuck
Burmy
Wormadam
Castform
Arceus

There is also the Spiky-Eared Pichu from a special event.
Also, for Cherrim, Unown, Burmy, Basurao and Pichu the form change is only cosmetic, but for the others it changes type or stats.
