As soon as you enter any Pokemon Center on your birthday in X and Y, the lights go dim. Then, all of a sudden, streamers fly and decorative ribbons fall from the top of the screen, while the Happy Birthday song plays in the same instrumentation as the regular Pokemon Center music. Nurse Joy exclaims, "Happy Birthday to you, insert name here!" A birthday cake with candles appears on the overhead screen where your Pokemon usually appear while they are being healed. Nurse Joy says a few more words wishing you well before healing your Pokemon as usual. The Happy Birthday music will continue to play any time you enter any Pokemon Center on your birthday.

Also, in the PSS, your avatar has a birthday cake over their head, and other passerby can wish you a happy birthday.

Finally, diamond dust falls on Route 17 outside Frost Cavern instead of regular snow.

Well, from what I've heard so far they will celebrate your birthday in the Pokemon Center. It's nothing too big. When you enter the Pokemon Center on the day of your birthday, of course, the lights would immediately go off and Nurse Joy will wish you a happy birthday, a birthday cake will also be shown on the big blue screen. That's pretty much it. It's a small little event, but cute.
