Are all of the Nurse Joys and Officer Jennys clones?

Why do all the Joys look alike and are nurses, and why do all the Jenny's become Officers as well as look identical?

This design choice is a joke. The games came out first, and had limited sprite models. As such, they recycled a lot of sprites. This is why they made all Joys and Jennys identical.

I don't know about Nurse Joy family but if you look a closer to the hat of a Officer Jenny, you will see all the Officer Jenny's are different.

They're all twins or cousins and like Solember said below limited sprite but then they kept it as a joke also because with the show its like: another nurse joy?! So yea joke and limited sprites.

Also they have different hats sooo....

No. Even though they are identical, they change appearances in every region. For example, a Sun and Moon Nurse Joy would have a different hair style than a Sword and Shield Nurse Joy. Also take note that they all fight over which one is prettier. Spoiler: none of them.
