I can't read any QR codes?

I keep trying to read some codes off of websites, but it just says that the qr code is unable to read. Could I get some help or some tips on how to scan the code right?

Make sure you're following the tips on the screen, as well as:
-be in a well lighted area
-hold the DS the set amount of length away from the qr code

When I needed to update something and use a qr code, it worked perfectly. Just make sure it fits within the square on screen.
