What exactly are hacked pokémon?

I have seen many questions, but not one on what hacked Pokemon exactly are? How do they pop up? Stuff like that!

Hacked Pokémon are Pokémon that were added into the game not by standard methods examples of standard methods being through trades, hatching, or encounters. Most hacked Pokémon are manually generated by someone.

Basically, a non-hacked Pokémon is one generated by the game naturally.
A hacked one is a Pokémon “man-made”.

According to the official Play! Pokémon VGC rulebook, illegally manipulated Pokémon (which fans often call a "hacked" or "genned" Pokémon) is when a Pokémon has been created or modified through the use of external, 3rd-party devices (section 4.3). Notably, this definition includes Pokémon with possible stats/moves that weren't caught/trained naturally through the games.
