How do entry hazards take effect in rotation battles?
Do the hazards trigger every time you rotate?

No

Stealth Rocks/Spikes only activated when a Pokemon is switched in and not rotated. But actually entry hazards aren't recommended in anything except single battles.
And the decrease from Sticky Web will come into effect when a Pokemon is switched in and will be in effect till the Pokemon is switched out in rotation battles and rotating does not affect the stat decrease.
